WE, FELY B. PASCUA and MARILYN O. TULIAO, of legal ages,
Filipino citizens and residents of Sitio Macondry, Barangay Maasin, San
Clemente, Tarlac, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and state THAT:

We both personally know RESURECCION CASTILLO


HERNAEZ (Resureccion, for brevity) who was born on May 24,
1948 at Bagonbayan, San Jose City, Nueva Ecija, Philippines
to Spouses Aniceto Hernaez and Trinidad Castillo;

Resureccion is also known as MRS. RESURECCION


HERNAEZ LAMBINICIO after she got married to Bernabe
Lambinicio;

We know the fact of birth of Resureccion since we are her


longtime family friends;

We are executing this affidavit to attest to the fact that


RESURECCION CASTILLO HERNAEZ aka MRS. RESURECCION
HERNAEZ LAMBINICIO was born on May 24, 1948 at
Bagonbayan, San Jose City, Nueva Ecija, Philippines and for
whatever legal purposes and intents it may serve.
