Offer of testimony:
To prove that she is a Record Officer IV of the Philippine Statistics
Authority (formerly National Statistics Office), Quezon City Branch.
That on February 25, 2015 in connection with her duty, she received a
copy of the Marriage Contract duly filled out, signed and contracted
between NEIL IAN RAMOS and RICA JANE ANGELES.
She will testify as to other matters that have a relation on the
existence and her issuance of the CERTIFICATE OF MARRIAGE
contracted between the accused and Rica Jane Angeles.

Since 1994 up to date.

A6. Doing clerical and admistrative works including the typing, input
of applications for birth, death and marriage certificates. But now I am
more on the management and administration of the Office.

As Administrative Aide I, contract of service for one year from
July 1994 up to July 1995
And how many years you worked in the Philippine Statistics
Authority, Quezon City Branch before you promoted to your
current position?
20 years.
Do you personally sign the Certificates of Marriages?
No Sir, it is being signed by our Head Officer once the
requirements are duly complete and signed by all parties
concerned.
Q10. Once the Certificate of Marriage is signed by your Head Officer is
it presumed valid and duly registered?
A10. Yes. It is also presumed to have been duly registered in the Local
Civil Registry.
Q11. I am presenting to you this Certificate of Marriage, can you verify
if it is deemed valid?
A11. Yes it is deemed valid. It bears all the requisites to consider it
valid.
Q12. What made you say that it is valid?
A12. It bears a registry number, the name and signature of the
contracting parties, the name and signature of the solemnizing
officer, the name of the witnesses and the name and signature of
our Head Officer at the bottom part signifying the validity of that
Certificate of Marriage.
Q13. What implication does this Certificate of Marriage signifies?
A13. That the contracting parties named in that Certificate of Marriage
is for all purposes are considered to be as husbands and wives.

one who conducted the examination of the herein affiant. I faithfully
recorded the questions I asked from the affiant and the affiant
