WHEREAS, the Second Party hereby possesses the education, experience and skills required to perform
the job as described herein;
WHEREAS, the Second Party hereby attests that (1) he/she is not related within the third degree of
consanguinity or affinity to the hiring authority and/or representative of the First Party; 2) that he/she has not
been previously dismissed from government service by reason of an administrative offense; 3) that he/she has not
already reached the compulsory retirement age of sixty-five (65);

The DEPARTMENT OF HEALTH (DOH), an executive department of the Republic of the Philippines,
created pursuant to Executive Order (E.O.) No. 94, s. 1947, transformed under E. O. No. 102, s. 1999, with office at
QMMC Compound, Project 4, Quezon City, herein represented by EDUARDO C. JANAIRO, MD, MPH, CESO
III, Director IV, DOH RO IV-B (MIMAROPA), hereinafter referred to as the First Party;

SEVENTY EIGHT PESOS (P26, 878.00); inclusive of taxes, payable once a month. Cutoff date is end of
the month. Salary shall be processed for payment after the cut-off period and upon submission of
complete supporting documents chargeable against the funds of Doctors to the Barrios and Rural
Health Practice Program (DTTB-RHPP), this Department, subject to the usual government accounting
and auditing rules and regulations;

2. The Second Party will receive a PhilHealth premium of P600/quarter, and P500/year GSIS Group
Accident Insurance coverage;

3. Open their Land Bank Account before the first salary cutoff of the semester for their payroll processing;
4. The Second Party shall be under technical/administrative supervision of the Municipal Health Officer or
the designated officer-in-charge and under general supervision of the Development Management Officer
(DMO);

5. The Second Party shall perform but not necessarily be limited to the following works:
a.

Render and complete the eight (8) working hours per day

b.

Can be assigned to BEMONC for night shifts considering the following terms:
i. They will be performing their duty in the presence of a PHN/Midwife;
ii. Each Nurse under NDP will only be assigned twice a month;

Assists in the implementation of various health programs of the LGU in accordance with the program
goals and objectives of the Department of Health and the Health Sector;

d.

Provides technical assistance in the implementation of the health programs;

e.

Assists in the conduct of disease surveillance;

f.

Conducts health education and training;

g.

Assists in the supervision and monitoring of the Community Health Team (CHT);

h.

Assists in the conduct of regular monitoring and evaluation of the various health programs for the
community:

i.

Assists in the preparation and analysis of reports required by the Regional Office/LGU regarding health
programs, clinic and community activities;

j.

Performs Clinical Functions such as:


i. Transport of patient and administer injection/immunization considering that he/she will
perform the tasks in presence/supervision of MHO/ PHN/Midwife;
ii. Observes, records and reports patients symptoms and conditions and progress of treatment and
therapy;
iii. Maintains records reflecting patient condition, medication and treatment.

k.

Structurally organize the local dissemination process from BHS to the core hospital level using health
resource augmentation in their respective areas.

l.

Disseminate the following information:


i. For patients/ clients going to the BHS requiring protocol for hypertension and DM refer to
RHUs.
ii. At RHU level, all patients referred and walk- ins requiring HPN/DM protocols shall undergo the
following:

Evaluate the health status of the patients (e.g. BP, capillary test, etc.).
Educate on Health Lifestyle.
Enlist the patient and provide the old Compack medicines if available and/ or schedule
them for a return visit for free medications once drugs are available.

iii. For those patients currently using Non-Communicable Diseases maintenance drugs with
doctors prescriptions that shall be included in the Health Club.
iv. For those requesting to change drug regiments to the available NCD drugs shall secure first a
certificate from their Physicians and shall thereafter be a member of the Health Club.
m. Perform Roles on Service Delivery Network:
i. Information dissemination regarding client registration with facility.
ii. Provision on health education, counseling, health promotion, and other related health services.
iii. NDPs shall monitor and report the progress of service utilization of the priority population
from the demands generated by the Community Health Teams. Reports will be submitted to
the Public Health Associates in the province.

Submits the following to the Provincial DOH Office:


i. Properly accomplished Daily Time Record (DTR) and accomplishment reports/journal signed
by the MHO/DTTB and DMO and shall be submitted every 5 th day of the succeeding month to
the Provincial DOH Office;
ii. Necessary documents and records for purpose of compensation, program monitoring and
evaluation as maybe required by the regional/PDOH Office.

o.

Abide the terms and conditions in the signed Code of Discipline;

6. Performs other related functions as may be assigned.


7. The Second Party shall perform work at a time and schedule to be agreed upon by both parties;

8. In the event that the Second Party is required in the interest of the service to undertake travels,
transportation expenses incidental to the travel shall be charged to the funds of the Doctors to the
Barrios and Rural Health Practice Program (DTTB-RHPP) subject to the availability of funds, usual
accounting and auditing rules and regulations;

9. That the First Party reserves the right to terminate this Contract on the following grounds;

Violation by the Second Party of the Terms and Conditions of this Contract;
Non-availability of funds;
Violation of program rules and regulations; and
Unsatisfactory performance of duties and responsibilities.
