1. I hereby enclose the following supporting documents together with the Application Form:
a) One (1) clear photocopy of the Contestants Malaysian Identity Card (new) and valid
Malaysian International Passport;
b) Three (3) recent 4R-sized photographs of the Contestant (1 without make up, 1 with light
make up and 1 with full make up) of the Contestant;); and
c) One (1) copy of the Contestants Sijil Pelajaran Malaysia (SPM) certificate or higher
qualification certificate.
d) A copy of DVD of Contestants performance video clip
2. I hereby confirm as follows:a) that I am free from any contractual obligation/arrangement or any other legal encumbrance. I
am not bound by any contracts, covenants or obligations with any recording company,
management company and/or any other companies or persons which may prevent me from
participating in this Competition and/or entering into a service/management/record label
contract with the Organizer and/or any of its affiliated and/or related companies;
b) that I have not appointed anyone to represent me as my Manager, Personal Assistant or
representative for any possible service contracts and/or arrangements.
c) that I am not an Astro Malaysia Holdings Berhad (Astro) and Adonis Beauty Consultants
Sdn Bhd (Adonis) employee and I do not have any immediate families/relatives who are
employees of Astro and Adonis and/or any of its affiliated and/or related companies;
d) that I have not been convicted of any criminal offence or under any pending investigations or
charges for any criminal offence; (please state if otherwise)

Competition including the mechanisms thereof and unconditionally accept and agree to

accurate and complete in all respect and shall remain true, and we have not will fully or
knowingly omitted and/or misrepresented any information. If I am notified by the Organizer
that I have been disqualified at any time during the Contest, I will withdraw myself from the
Contest and/or the production thereof and will not challenge or question such decisions.
g) that I agree that I have read, understood and agreed to the terms of this Privacy Notice
attached herein and consent to the processing of my personal data as described in the
privacy notice.
