It was observed that no any safety equipments (safety Jackets, Helmets, etc)
provided to staff working in Culvert reconstruction at 57+010 from 01-122013. Hence arrange safety equipments for all staff work in Construction
immediately. Also you are advised to follow "Safety Inspection Form" in Site
instruction No. 02. Also you are required to follow weightings for monitoring
safety arrangement as per the Contract Document (Ref. Page 069).
