I understand that opting my student out of testing may have the following consequences:

When students do not test there is limited assessment information for teachers to create
instructional plans for your student.
Not having accurate assessment information for all students in the school hinders school
improvement planning. School improvement helps to make sure the school has educational
services and programs to meet the needs of all students.
If less than 95% of the students test the School Report Card letter grade will be lowered by one
grade.
Teachers evaluation ratings from the Public Education Department may be affected if students in
their class do not test.

I have read the information above and I am requesting my student not participate in the following state
mandated or district required testing for 2015-16.

NMAPA/NCSC (Alternate for Special Education Students)
PARCC (Partnership for Assessment of Readiness for College and Careers) (English
and Math achievement assessment; used for school grades & teacher evaluation)
(English Language arts/literacy: 9th 11th, Math : Algebra I, II, & Geometry)
Standard graduation requirements include passing scores on PARCC English
11, Geometry or Algebra II
Interim Assessment (9th, 10th ELA, Algebra I & Geometry)

By signing below you are confirming that you have read or discussed with an APS representative the possible
consequence that of opting out of testing may have for your student, your students teacher and your students
school.
I also understand that opting out my student from high school tests that are required for graduation may result in
my student not earning a high school diploma.
