APPLICATION FOR SECURITY CLEARANCE FOR ACCESS TO
CLASSIFIED MATTER
INTRUCTIONS:
1. Pursuant to PNP Security Policy, All PNP personnel who have need for
access to PNP clasified matter shall be required to obtain clearance from the
Directorate for Intelligence, PNP and other authorized clearing authorities. This form is
intended to expedite the issuance of such clearance.
2. Attached the personal history statement duly administered by notary
public/official authorized to administer oath.
3. Joint security statement from 2 disinterested employyes who have at least
3 years continous service is required (PART B).
4. This application shall be endorsed by official communicationby the head of
the officer or unit through channels by mail or courier to DI, PNP (Attn; CSD), Camp
Crame, Quezon City or authorized clearing agency in 2 copies.
5. In order to facilitate the issuance of clearance, the head of office or unit
should endeavor to ensure that the applicant has no involvement in activities inimical to
the security of the state.
