o(erating integrit/ and the safe9 continuous and effective o(eration of the facilit/ /
station.

case -here a cre- change ta&es (lace.
<andover notes that are not su((orted ./ clear and concise Dver.alE co$$unication
often leads to $isunderstanding as to the Dtrue statusE .eing conve/ed.
!isinter(retation of the handover docu$ents $a/ result .ecause each individuals
infor$ation needs are different. )lti$atel/ these $a/ i$(act u(on asset (erfor$ance
through $issed .usiness o((ortunities and un(lanned defer$ents.
To address the a.ove all 6afet/ ritical +(erations (ersonnel shall conduct a for$al
handover -ith .oth -ritten and ver.al actions9 and -here (ossi.le .e of a $ini$u$
t-o hour duration. ?here re@uired the handover -ill .e su((le$ented ./ hec&lists
and <andover 6heets.
1#! Purpose
The (ur(ose of this (rocedure is to (rovide direction for +(erations (ersonnel9 in all
areas9 -ho are re@uired to carr/out cre- changeovers. The re@uire$ent of handover
-ill .e ver.al co$$unication su((le$ented ./ a -ritten handover re(ort. The
res(onsi.ilit/ -ill .e on .oth (arties9 outgoing and inco$ing9 to ensure that the
handover ensures an accurate transfer of infor$ation9 -ith sufficient clarit/ to avoid
a$.iguities.

5dherence to this (rocedure is mandator for all (ersonnel -ho are re@uired to
handover to their o((osite nu$.ers 1onco$ing cre-3 infor$ation that is safet/ critical
and could affect the safe o(eration of the facilit/ / station.
The (rocedure shall cover nor$al rotational or eBtended cre- changes and additionall/
an/ -or&s .eing carried out over a (rolonged (eriod that involve the -or&ing of $ore
than one co$(lete cre-9 such as $aintenance ca$(aigns9 -ell service -or&s etc9.
<andover notes / instructions shall .e entered on to the a((ro(riate "or$ and archived
for future reference on co$(letion of the handover. This shall ensure that (ersonnel in
&e/ (ositions shall .e a.le to reference historical as -ell as current infor$ation
regarding -or& in (rogress or co$(leted. The archived infor$ation shall .e accessi.le
to all (ersonnel. o$(liance shall ensure that the records can .e audited.

use during the handover (rocess. The for$ ensures that all sa5et critical infor$ation
is ca(tured and docu$ented in a for$at that is easil/ understood ./ the inco$ing
(art/. The for$ is an integral (art of the handover (rocess.
The +(erations 6afet/ ritical <andover "or$ is raised ./ the outgoing (art/ and is
used in conFunction -ith the for$al ver.al handover to ensure the inco$ing (art/ is
$ade a-are of changes or activities -hich could have an i$$ediate i$(act u(on the
safe o(eration of (rocesses and e@ui($ent -ithin their area of res(onsi.ilit/.
+n co$(letion of the handover9 including a site ins(ection if re@uired9 .oth outgoing
and inco$ing (arties shall ac&no-ledge this transfer of infor$ation ./ signing the for$

underta&en -hich does not eBactl/ fit a categor/ listed in 6ection ,.5 of PR-11729 or
-here a concern has .een identified and recorded.

This section is used to list the -or& in (rogress -here isolations are still in (lace and

safet/.
The sections included on the "or$ should in the $ost (art address the needs .ut eBtra
Dto(icsE can .e included as re@uired. The notes are intended to su((ort the handover
(rocess ./ (roviding reference infor$ation. +n co$(letion the "or$ shall .e attached
to the +(eration 6afet/ ritical <andover "or$.
The (erson co$(iling the "or$ shall decide the level of detail re@uired ta&ing in to
consideration that the "or$ is intended to .e used in conFunction -ith a ver.al for$al
handover. =n general .ulleted (oints should .e used to stress the ite$s that are
