Constipation Nursing Care Plan

After assessing a patient experiencing constipation due to decreased dietary intake, the nurse implemented several interventions over 8 hours to help establish normal bowel functioning: 1) The nurse monitored stool characteristics, auscultated bowel sounds, and encouraged fluid intake to identify causative factors and appropriately treat the constipation. 2) Interventions included recommending increased fluid and fiber intake, avoiding gas-forming foods, and assisting with perianal skin care. 3) The nurse also discussed the use of stool softeners or laxatives as needed and encouraged eating more high-fiber foods while collaborating with a dietitian to provide a well-balanced, high-fiber diet.
