1.0 Project Background
Andhra Pradesh (AP) state has one of the longest coastal belts in India. Out of 67,362 Km of road network in the state, the nine coastal districts cover 26,000 km of road length. Every year during the period of April-May and September-December, cyclonic situations set in to the Bay of Bengal. With the recent changes in climatic conditions, the catastrophic events are increasing resulting in very devastating cyclones like Laila, Jal etc where the loss is enormous. In the 97 years ranging from 1892 to 1989, there were as many as 67 cyclones within the State. It is essential to provide safe residential accommodation to the vulnerable population during any eventuality or disaster due to natural calamities such as cyclones and associated tidal waves and storm surges. The project is formulated based on experiences of Government of Andhra Pradesh in mitigating such disasters during the past and also keeping in view the existing facilities available in the project area in Andhra Pradesh. The Government of India has applied for a credit of US$ 255 Million equivalent from the International Development Association (IDA) toward the cost of National Cyclone Risk Mitigation Project (NCRMP), and it intends to apply the proceeds to payments for goods, works, related services and consulting services under this Project. National Cyclone Risk Mitigation Project (NCRMP) Project Implementation Unit (PIU), Andhra Pradesh, headquartered at Hyderabad has invited the consultants to submit proposals for providing the consultancy services for Third Party Quality Audit for 9 coastal districts namely Srikakulam, Vijayanagaram, Visakhapatnam, East Godavari, West Godavari, Krishna, Guntur, Prakasam and Nellore. The project Construction of Multipurpose Cyclone Shelters, Roads to Cyclone Shelters, Roads to habitations and Roads & Bridges under NCRMP is funded by International Development Association and received through Government of India. This consultancy is envisaged to audit the quality of works being implemented by various line departments working under PIU. The number of works under progress / for which agreements are concluded in each district is shown in the table below.

The district wise progress of works is shown below. The works in the West Godavari are not started and the works in Krishna District has just started. At many places the first milestone is completed but works are not started. The line departments are to analyze the reasons for the slow progress and if necessary penalty for the slow progress as per the agreement conditions should be imposed.

6.0 District wise status of tests conducted by TPQA: Apart from monitoring the tests being conducted by the implementation team (contractors & Line department) at various stages, the Third party Quality Audit team also conducted tests to confirm its compliance with the standards. These tests are carried out in the site itself and in some cases samples are collected and testing done in outside laboratories. The details of tests conducted in the field till end of September 2011 are presented.

It is fairly tallying with the specification of clause 405.13 of MORD. QC tests conducted for WBM GRADE II &III (field and lab) i.e. Gradation, AIV, FIV and water absorption. The details are given in QC Test summary. It is fairly tallying with the specification of clause 400-7 of MORD Guard stones are to be along the outer edge of sharp curves and White wash is to be done to stones for the clear visibility to vehicular traffic. CD Works: 1.) Pipe culverts 600mm dia 2no.s in KM 0/2 & Km.1/6 and they are not yet started. From Km.1.930 to Km.1.980 50m CC road is to be done. Village name of work boards, cautionary and mandatory signboards at necessary points at school locations shall be erected as per the NCRMP norms. As per agreement it is agreed that two technical agents will be provided by Contractor on his works and the names provided are Mr. G.Manjunath Reddy and Mr. C.Mallikarjuna. No technical agent is available at site during the inspection. Para wise compliance report shall be furnished duly attending to the above remarks. Pipe culverts 600mm dia 2no.s in KM 0/2 & Km.1/8. Village people were requested to provide extra 2 nos. 0f pipe culverts in Km. 0/2 & 1/4 as per the field officers information. Village name boards, cautionary and mandatory signboards at necessary points and at school locations shall be erected as per the NCRMP norms. As per agreement it is agreed that two technical agents will be provided by Contractor on his works and the names provided are Mr. G.Manjunath Reddy and Mr. C.Mallikarjuna. No technical agent is available at site during the inspection. Para wise compliance report shall be furnished duly attending to the above remarks. ROAD FROM PR ROAD TO DASARIVANIPETA AND TO CYCLONE SHELTER 18% value of the work is completed. WBM grade II works is in progress. As per the Agreement the Mile stones fixed are as follows. Milestone 1 to the end of Dt.30.09.2011 Earth work, CD WORKS, GSB Milestone 2 to the end of Dt.30.11.2011 , WBM Gr II and Gr III and Shoulders Milestone 3 to the end of 20.01.2012 BT and Road Furniture. The measurements like width and thickness of the GSB Layer from Km 0.000 to Km 1.400 is verified and found to be fairly tallying with the required measurements. FDD, MDD,OMC,CBR and Plasticity index tests conducted for GSB layer in the same reach (JNTUK) and they are tallying with the agreement requirements. The details are given in Annexure 2 and in QC test summary from km.1.400 to Km 2.280; the work could not be started due to Land Acquisition problem. The measurements like width and thickness of the WBM Gr.-II Layer from Km 0.000 to Km 1.400 is verified and found fairly tallying with the required measurements. The details are given in Annexure 2. Volumetric analysis tests were conducted for WBM grade II layer from Km 0.000 to Km 1.400. It is fairly tallying with the specification of clause 405.13 of MORD. QC tests were conducted for WBM GRADE II (field and lab) i.e. Gradation, AIV, FIV and water absorption. The details are given in QC Test summary and they satisfy the MORD requirements. Guard stones are to be fixed Curved areas and White wash is to be done to stones for the clear visibility to vehicular traffic. Widening the formation is not done as per requirements and it is less than 7.5M at almost all the locations. Lateral support to the road crest is very important.

7 CD Works: In KM 2/2, 2M slab culvert is completed. Approaches are formed with BC Soil. It is to be removed; approaches shall be formed in uniform layers with 98% relative compaction. Only granular material is to be used. Pipes are laid in KM 0/4 in a skew direction. Body walls are to be constructed. At this location 7.5M width is not sufficient. Extra Width is necessary at this location. Village name of work boards, cautionary and mandatory signboards at necessary points all along the outer edge of sharp curves and at school locations shall be erected as per the NCRMP norms. As per agreement it is agreed that two technical agents will be provided by Contractor on his works and the names provided are Mr. Kaki Suresh and Mr. Aienaparthi veeranna. No technical agent is available at site during the inspection. No proper diversion boards provided at the entry and exit of the culvert locations. Diversion boards and barricades around the excavations need to be provided. Para wise compliance report shall be furnished duly attending to the above remarks. PROVIDING BT SURFACE TO THE ROAD FROM R &B ROAD TO AVARAJAM IN ACHUTAPURAM MANDAL 31.00 % value of the work is completed. Shoulder works are in progress. Works to be done as per Milestone: Mile stone 1 to the end of Dt: 26.07.11- Earthwork, CD Works and GSB. Mile stone 2 to the end of Dt: 26.10.11- WBM Gr-II, Gr-III and Shoulders. Mile stone 3 to the end of Dt: 26.01.12- BT and Road furniture. The formation widths and Earthen shoulder widths are verified from Km 0/0 to Km 1/110 and at some locations shoulders have slipped and the same shall be rectified. It is observed that the shoulder material of sand and gravel is not properly mixed. It shall be scarified, suitably remixed, watered to OMC and compacted to achieve the required relative compaction of min.98% of lab MDD as per the agreement clause. The approved material in shoulders was spread and sectioned at some locations; the formed width is less than the approved drawing width 7.5m. The formed width is varying from 6.0 to 7.5m. When asked about the above deficiency in width the field officers have stated that due to non co-operation of adjacent building and land owners. It is not possible to make up the road width. The measurements like width and thickness of the WBM Gr.-3 Layer from Km 0/0 to Km 1/110 is verified and found fairly tallying with the required measurements. The details are given in Annexure - 2. Volumetric analysis tests conducted for WBM layer from Km 0/0 to Km 0/110. It is fairly tallying with the specification of clause 405.13 of MORD. Whitewash to be done to parapet walls of the existing culverts for the clear visibility of vehicular traffic. The measurements like width and length of parapets and pipes of all the culverts are verified and found tallying with the estimate. Some of the KM and HM Stones are missing in this working road stretch. The field officers are requested to make arrangement to rectify them immediately. Village name boards, cautionary and mandatory signboards at necessary points all along the outer edge of sharp curves and at school locations shall be erected. 13 Review of degree of Quality Control exercised: The summary QC tests by field officers shown under 3.0 of this QCR are noted from the following: 1) Test records shown by line departmental officers. 2) Test results like CBR and Plasticity Index etc., obtained from local testing facilities. This aspect requires further improvement and commitment of senior engineers such as SEs and EEs. Setting up of a QC lab for conducting most of the regular tests for each package by the contractor is necessary. The line departmental officers also require one laboratory for each sub-division or Division depending on the workload for ensuring better quality control. In the present case, no Quality Control laboratory was established by the contractor. No QC laboratory is available at sub-division or Division. Name of Work: 1 2 3 4 Construction of Minor Bridge at Km. 15/2 near Chakaligadda of Konada Ramatheertham R&B Road to Konathalapalem (via) Yerukonda, Pasupam, Kollayavalasa (Z.P. Road) in Vizianagaram District. Progress: About 66.19% of work is completed. The field officers have furnished the test results for Water, steel and cement obtained from Department of Civil Engineering, Andhra University, Visakhapatnam. The field officers are also requested to apply cement slurry to the steel bars so as to protect from rusting. The field officers are also requested to test the samples of cement from every consignment, conduct all the required tests on cement such as cube strength, initial setting time, Final setting time, consistency, soundness etc. The field officers are requested to provide the invoices bills of the materials for verification. The reinforcement steel shall be neatly stored on wooden blocks, racks or platforms above the ground in clean and suitably marked to facilitate inspection and identification as per Cl.1603 of MORTH. Caution boards and sign boards are to be kept. Safety measures for workers such as helmets, gum shoes, jackets etc., are to be provided by the contractor. Construction of Minor Bridge at Km. 7/2 near Puligadda of Konada Ramatheertham R&B Road to Konathalapalem via Yerukonda, Pasupam, Kollayavalasa (Z.P. Road) in Vizianagaram District Progress: About 30.96% of work is completed as against 40.54%. The progress of work is slightly slow. Sieve analysis, AIV and FI is done for Coarse aggregate and SA, FM and Silt content for Fine aggregate the result is found within the specified limits as per MORD Table 400-7. Road from R&B Road to Patuvari Kandriga In Nellore District. Works to be done as per Milestone: Mile stone 1:- Earth work, pipe culverts, GSB: 3 months. Mile stone 2:- WBM Gr-II, Gr-III and Shoulders: 6 months. Mile stone 3:- BT and Road furniture: 9 months. 64% value of the work is completed. GSB widths are verified from KM 0/0 to 0/470 and found to be tallying with the required measurement. The details are given in annexure-I. WBM Gr-II, WBM Gr-III widths are verified from KM 0/0 to 0/470 and found to be tallying with the required measurement. The details are given in annexure-2. At KM 0/160 on the R/S shoulder existing water sump should be relocated. The Field officers are requested to provide village name boards, sign boards & caution boards. As per estimate provisions OGPC pavement is from 0/0 to 0/575 km and CC pavement is from 0/575 to 0/775 km. But OGPC is laid upto 0/470km only as the habitations are starting from 0/470km, and residents had stopped further laying of OGPC beyond 0/470. The remaining 105m CC pavement is to be laid extra in addition to CC pavement length as per the estimate provisions. Samples were collected from the layed OGPC surface by excavating pits and 4 samples of 500g were tested for BT content. The Percentage BT was found to be 4.35% as against the required 4.235% (0.3%). BT requirement is calculated on percentage basis as per IRC: SP: 20 and IRC: SP: 11. Rain Gullies formed in the embankment at some locations in Km 0/0 to0/470. These are to be filled, compacted and trimmed to confirm the specified side slopes. Further progress of work is stopped for want of clearance of 105m length CC pavement from Km 0.470 to Km 0.575.
