1. FILL OUT NEONATAL ALERT FORM 2. OBTAIN CURRENT U/S REPORT WITH CURRENT SUMMARY 3. FAX NEONATAL ALERT FORM & U/S REPORT TO 404-851-8162 (for input into Image Freeway) 4. FAX NEONATAL ALERT FORM & U/S REPORT TO 678-990-5763 (for Dr. Kupke) 5. PLACE NEONATAL ALERT FORM, FAX VERIFICATIONS & U/S REPORT IN PATIENTS CHART 6. STAMP NEONATAL ALERT AT THE TOP OF CHART & FRONT CORNER 7. PLACE NEONATAL ALERT FORM IN LOG BOG (after confirming that it has been logged into Image Freeway takes a couple of days) 8. ALL NEONATAL FORMS WILL BE KEPT IN BINDER AT NSH ATLANTA LOCATION ALONG WITH A COPY AT THE INITIATED LOCATION 9. NURSE AT NSF WILL GENERATE NEONATAL ALERT FOR THE ALPHARETTA LOCATION To verify Neonatal Alert is in Prenatal System go to: 1. 2. 3. 4. 5. 6. 7. NORTHSIDE HOSPITAL INTRANET CLICK ON APPLICATIONS IMAGE FREEWAY: enter user name and password CLICK ON LABOR & DELIVERY CLICK ON SEARCH PROJECT ICON TYPE IN THE LAST NAME, FIRST NAME OF THE PATIENT GO TO SEARCH TO LOCATE THE NEONATAL ALERT
