II-Problem Presented: Subject was diagnosed to have a chronic renal failure and had a continuing medication. He started to have hemodialysis since January 2010. III-Findings: Subject is married Gene Ibanez of Jovellar, Oas, Albay only last August 1, 2009 and remain childless up to the present. He use to work as farmer tilling a 1/8 hectare riceland but after he got sick, he could no longer engaged in manual work. He is now dependent to his wife who is casual employee of Jovellar Municipal Health Office as RHU Midwife with a monthly salary of seven thousand pesos. His wife goes home once a week. The spouses are staying in his ancestral house made of lumber and cement. The family pays about P550.00 for electricity;P240.for water bills.They are are using LPG stove or sometimes use charcoal in cooking. Subject has maintenance medicines and is having hemodialysis 2 t 3 times a week and this is very costly.

IV-Evaluation and Recommendation:


Subject needs medical treatment but incapacitated to cover up the cost, he is now dependent to his wife who has low income. He is therefore recommended to avail of the financial assistance of the Philippine Charity Sweepstakes Office for his hemodialysis.
