Ankle Foot Orthoses

Ankle foot orthoses (AFOs) are bracing devices that are placed around the lower leg and foot to provide support and control of the ankle. There are several types of AFOs: 1. Static AFOs do not provide movement while articulating AFOs allow a regulated amount of movement in the ankle. 2. Functional AFOs include metal bar/conventional AFOs, total contact AFOs, floor reaction AFOs, unweighting AFOs, and immobilizing AFOs. Total contact AFOs provide better control through a sleek intimate fit while unweighting AFOs reduce pressure on the ankle using prosthetic principles.

ANKLE FOOT ORTHOSES

Swagatika
SMO

• Supra Maleolar
Orthosis
• Low profile design
that crosses the ankle
• Less invasive trim
lines than a standard
AFO
• Covers about 80% of
the surface area of
foot
TYPES
• Static Ankle foot orthoses
• Not solid AFO

• Articulated – that provides desired/ regulated

amount of movement
• It may also be used for progressive increases
or decreases in sagittal plane ROM and control
TYPES (functional)
• 1. Metal bars or conventional
• 2. Total Contact
• 3. Floor reaction
• 4. Un weighting
• 5. Immobilizing
Static AFO
Articulating AFO
Conventional/Metallic AFO
• Low cost
Disadvantages:-
Heavy
Contains mechanical stirrup,
ankle joints…wear and tear,
Is external ,hence less effective
Total Contact AFOs
• provide sleek, intimate fit with
total contact to provide better
control
• May be thermoplastic and
thermosetting
• higher patient acceptance due to
light weight & concealment (150-
200gms);
• may be hotter ,if not perforated
• more common today
Floor reaction AFO/FRO
• Uses floor
reaction force
through toe
aspect of foot
plate to prevent
forward tibial
progression &
subsequent knee
collapse
Unweighting AFO
• May be patella tendon bearing (PTB) ,i,e. specific
weight bearing

or

• total surface bearing, TSB (inverted cone with

lace closure) to unweight the ankle foot using
prosthetic principles
TSB unweighting AFO and specific
weight bearing AFO
Immobilizing AFO
• Commonly used with a lower extremity
deficiency when ankle immobilization is
desired
• – distal tibia/ fibula fracture
• – foot bone fractures
• – tendocalcaneus rupture
• – Diabetic Foot (Charcot Foot)
Immobilizing AFO
