Weekly Self Evaluation week 6

Considering your patients current status, list potential complications and
strategies for prevention and early recognition.
- This week I shadowed a home health nurse. The patients we seen
required different levels of care. Such as wound vac care, medication
management/adherence, wound care. One common factor would be
assistance. Assistance with home cleaning/ even respite for caregivers.
Those cg that I seen during my HH visits looked exhausted. I believe that
a program for respite like HUGs, would benefit for cg who are strained.
2. Am I getting more comfortable with the use of the nursing process to plan
and evaluate nursing care? (give examples of how it is better now or
problems that still bother you)
- Currently I do feel comfortable with the use of the nursing process. No
problems at this time.
3. Were my nursing diagnosis and plan of care individualized for my patients?
(Give examples of how you did this.) Do I have difficulty in the area?

- None done this week for HH
4. How are my assessment skills developing? Am I being as thorough as I need
to be? What areas are still difficult for me and what am I doing to improve?
(Be specific)
- I feel that my assessment skills are developing appropriate. I am still able
to notice changes in patients that may require the attention of the primary
nurse. This week being a working LPN with my experience with working
along side my facilities wound nurse; I noticed that there are a few
patients that would benefit with different would treatment aside from their
current care. Again respite for cg is very important.
5. What new skills did I implement this week? How did I do? What could have
helped me to improve? Did I ask for help when I needed it?
- Home health was a new thing for me. Especially since I used to be a
home health aide as well, its nice to see the nurse/medical side of home
health nursing.
6. How is my time management progressing? What areas of difficulty have I
found and what can I do to improve? How do I monitor my time management
while in the clinical area?
- I do have very good time management. Especially since I was able to help
the home health nurse with his patient care, this helped keep the timing
of nursing care at a reasonable time.
7. Was I involved in making referrals for my client in any way? How could the
nursing role in this process have been strengthened?
- Yes, I did suggest easier ways for the nurse to perform his dressing
changes. He really appreciated the suggestions and did it the way I
suggested to him with the next few patients and agreed it was a very
good idea.
8. List the specific interventions, in order of priority, for two of your clients and
explain how you determined which interventions took precedent.
- Home health nursing is seen on a set schedule. If there are any issues
that require immediate attention, the patients are encouraged to seek
immediate medical help by either call 911, or going to the ER.
9. Did I identify and report unsafe occurrences in client care? (Describe)
- For Home health nursing: there was a patient was in a hospital bed that was not safe. The
functions for the bed did not work. This placed the patient at risk for aspiration as the patient
is considered total care with ADLs. I pointed it out to the home health nurse and he did call
the SW and make a referral for proper health care equipment.
