THIS NOTICE is to inform you that your application to rent the property at
_______________ [rental property address] has been denied because of [check all that
apply]:
[ ] Insufficient information in the credit report provided by ________________ [name,
address and telephone number of consumer credit reporting agency providing credit
report; including toll-free telephone number if this is a national credit reporting agency]
[ ] Negative information in the credit report provided by ________________ [name,
address and telephone number of consumer credit reporting agency providing credit
report; including toll-free telephone number if this is a national credit reporting agency]
The consumer credit reporting agency noted above did not make the decision not to rent
to you and cannot explain why your application was rejected. It only provided
information about your credit history.
Providing this notice meets the requirements of the federal Fair Credit Reporting Act (15
U.S.C. 1681 and following). You have the right under this Act to obtain a free copy of
your credit report from the consumer credit reporting agency named above, if your
request is made within 60 days of this notice (15 U.S.C. 1681j). You also have the right
to dispute the accuracy or completeness of your credit report and add your own
consumer statement (up to 100 words) to the report (15 U.S.C. 1681i). For more
information, contact the above-named consumer credit reporting agency.
