Dear Dr Alexander,
Re:
Freedom of Information request (Our Ref: FOI009172)
Thank you for your Freedom of Information (FOI) request
dated 20 November 2015.
Your exact request was:-
1) the total cost of all the published independent
investigations
2) the range of costs, in terms of the greatest and the smallest
cost
3) the average cost of these investigations
In a separate email of the same date you also asked NHS
England:
May I ask where in NHS Englands accounts does the cost of
these independent investigations feature, presuming that NHS
England declares these costs?
I could not find any data on the cost of independent
investigations on the NHS England list of spending over 25K:

NHS England holds some of this information. For your
convenience we have responded to each of your numbered
questions in turn below.
1) Of the 60 Independent Investigation reports published on
NHS Englands website, 41 were commissioned by former
Strategic Health Authorities (SHAs). NHS England does not
hold the financial information associated with those cases.
The total costs of the remaining published 19 Independent
Investigations commissioned by NHS England for the period
2013 2015 is 543,555.00
2) The range of costs for these Investigations is 22,000
74,215. NHS England is unable to comment on what costs
within this range are associated with specific cases.
3) The average cost of the 19 published Independent
Investigations 2013 -2015 based on the total costings
provided above is 28,608.16. The national budget for
Independent Investigations for 2015/2016 is 1,000,000. This
is based on an assumed average cost of 33,000 per
investigation.
The budget for Independent Investigations is part of an
ongoing Directorate work programme budget and therefore
Independent Investigation costs do not feature in the list of
spending over 25,000.
