Endophthalmitis Case in CKD Patient

A 29-year-old male presented with pain, swelling, blurry vision and redness in his right eye for 3 days. He has a history of chronic kidney disease and hypertension. On examination, his right eye showed edema, spasm, hyperemia, chemosis, conjunctival vascular injection and a hazy view of the posterior chamber. He was assessed with right eye endophthalmitis and chronic kidney disease. The treatment plan included hospitalization, topical and intravenous medications, and monitoring of his intraocular pressure.

CASE REPORT

MS/MALE/29 YO

History taking:
Patient was consulted from internist with complain pain
and swelling on his right eye since 3 days ago. Patient felt
severe pain on right eye continuous along day until unable
to sleep. The pain only localized on right eye. Swollen
palpebral and difficulty to open it. Another complain was
blur vision, watery eye, and redness on right eye. Beside
that patient also got fever and felt pain on the right head.

Past History :

Theres no history of same symptom before. Patient have history


of Chronic Kidney Disease (CKD) stage V and hypertension for 1
years. Patient didnt have history of diabetes mellitus. No history
of eye surgery and trauma. No history of allergy to specific drugs
or foods. Patient didnt have history of using spectacles. Patient
has history of regular hemodialysis.
Family history :

None of the family member which suffer from the same condition
Social history : Patient work as farmer

Timol e.d 2x1 RE
Noxifloxacin e.d 1 drop @ 1 hour RE
Prednisolone acetate e.d 6x1 RE
Lyteers e.d 6x1 RE
Acetazolamide 2x250 mg tab
Ketorolac inj 2x30 mg iv (for 2 days)
