What does your school/staff hope to gain as a result of your participation in the FAME
2014-15 cohort? To foster self-reflection and dialogue around assessment taking
responsibility for our own professional practices.
What is the current level of collaboration between your teachers? We have an
expectation that teachers plan collaboratively weekly. Overall, collaboration is at a
moderate level, however we need to grow in developing our reflective practices.
What is the current level of implementation of the formative assessment process in
your school?

1- Not implemented
2- We are researching ways to implement formative assessment
3- We have partially implemented formative assessment
4- We are on our way to full implementation
5- Fully implemented the formative assessment process
What resources do you currently have in your school that support classroom formative
assessment practices? Teachers get an hour of paid collaboration time per week. We
hold 5 CFIPs per marking period where we look at classroom and school data. We have
developed learning experiences which have formative assessments embedded.
Teachers attend Math Institute (county PD) which expect teachers to share with
teammates best practices they learned and are applying.
