This is to certify that DR. ISAIAS M. CORTADO of Dupax del Norte National High School, Lamo,
Dupax del Norte, Nueva Vizcaya has been cleared from all money and property responsibilities.

This clearance is issued subject to the provision of GAO, General Rule Circular No. 52 dated December
23, 1975 part of which is quoted below.

If later an official employee who has been cleared is discovered still accountable for cash and/or
property, the clearance previously issued will not release him/her of said accountability.
