Viewing Form Templates vs. Starting New Forms

In Frontline, you are able to self-initiate several forms without needing to contact Central.

Click on the blue form title if you wish to review what the form is before starting.

Click on Start This Form if you wish to initiate this form.

NOTE: Clicking Start This Form will make your progress visible to Frontline Administrators in the Central Office. This means if you start the form but don't enter any information, or if you save hoping to return later, they may be reaching out to follow up as it appears in their dashboard under Incomplete Forms.

A list of available forms to start is displayed on a digital interface, with a purple tab indicating %22Forms I Can Start%22 at the top.
