Objectives This study aims to assess the prevalence of health problems (eg, insomnia, binge-eating, substance use and ill health) among UK doctors and to investigate whether occupational distress increases the risk of health problems. Design This study reports the analysis of data collected at the baseline stage of a randomised controlled trial (protocol #NCT02838290). setting Doctors were invited through medical Royal Colleges, the British Medical Association's research panel and a random selection of NHS trusts across various UK regions. Participants 417 UK doctors with an equivalent split of gender (48% males) and seniority (49% consultants). Main outcomes and measures Outcomes were sleep problems (eg, insomnia), alcohol/drug use (eg, binge-drinking), ill health (eg, backache) and binge-eating (eg, uncontrollable eating). Predictor variables were occupational distress (psychiatric morbidity, burnout, job effort, work-life imbalance, coping with stress through self-blame or substances) and work factors (workplace and years practising medicine). results 44% of doctors binge-drank and 5% met the criteria for alcohol dependence; 24%-29% experienced negative emotions after overeating and 8% had a binge-eating disorder; 20%-61% had some type of sleep problem and 12% had severe/moderate insomnia; 69% had fatigue and 19%-29% experienced other types of ill health problems. The results show that occupational distress and job factors increase the odds of doctors using substances, having sleep problems, presenting with frequent symptoms of ill health and binge-eating. For example, burnout increased the risk of all types of sleep problems, eg, difficulty falling/staying asleep, insomnia (OR ≥1.344; p≤0.036). Even taking into consideration whether or not a doctor works in a hospital, the risk of health problems still rises when doctors have signs of occupational distress. Conclusion Early recognition of occupational distress can prevent health problems among UK doctors that can reduce the quality of patient care because of sickness-related absence.

La evidencia en cada período de historia nacional demuestra que siempre hubo desarrollo salubrista en las diferentes épocas. La educación médica estuvo presente en nuestro país desde el comienzo de la etapa colonial transcurriendo por diferentes períodos con un carácter evolutivo, experimentando modificaciones esenciales para el perfeccionamiento. No se pudiera hablar de educación médica cubana sin la presencia a lo largo de la historia de grandes médicos-pedagogos encargados de ingeniar los diferentes sistemas de enseñanzas. Fue el profesor Fidel llizástigui Dupuy, paradigma de las ciencias clínicas, y más que ello, el pedagogo más trascendental de la medicina cubana. Es por ello que se realizó una revisión de 14 referencias bibliográficas con el objetivo de argumentar la importancia de la obra del profesor Ilizátigui Dupuy en función de la formación de profesionales en salud. The evidence in each period of national history shows that development always were public health professionals at different times. Medical education was present in our country since the beginning of the colonial period wore on for different periods with an evolutionary character, experiencing essential modifications in the course. The law and medical professions have long been seen as prestigious career paths and great options for adolescents aspiring for higher education. Both professions command attention and are highly competitive in Nigerian universities. However, the counseling need of adolescents wishing to study law or medicine differs. While counselling of adolescents aspiring to become lawyers (attorneys) prioritizes analytical, oratory, communication and public speaking skills, the counseling need of adolescents passionate to study medicine lay more emphasis on scientific ability, empathy, medical pedagogy, stress and medical management.

Researchers that are medical doctors (with or without additional degrees), need to be aware of the unique aspects that can affect their eligibility as ERC candidates. In this context, there are three critical issues to consider: 1. Medical doctor (MD) degrees do not automatically qualify as eligible degrees, 2. Applicants holding an MD should be aware of a possible shift of their eligibility window, and 3. Applicants that underwent clinical training may request a further extension of eligibility in certain cases. In this article, we will address these aspects in order to clarify the options that ERC candidates with medical doctor degrees may have in this regard.

Social implications-This study offers a new perspective on the negative effect of family interactions on work-domain outcomes. Originality/value-The paper extends the scholarly literature on stress and work-family interface by demonstrating that family incivility has spillover effects. This is the pioneering study that examines family incivility as a home demand causing long-term severe damages at work.

À la veille de la Première Guerre mondiale, l’armée britannique fait dépendre le recrutement des soldats d’une procédure de sélection qui repose sur une évaluation médicale de leurs capacités . À l’issue d’un examen anthropométrique et diagnostique, les individus considérés comme inaptes sont écartés du rang. L’expertise scientifique du recrutement est ainsi la garante de l’efficacité et de l’homogénéité du corps militaire, de l’adéquation entre les spécificités du candidat et les exigences du milieu militaire, et de la préservation de la santé des armées.
Or, dès l’entrée en guerre, le passage brutal d’une armée de métier aux dimensions réduites à une armée de masse, l’urgence propre à la mobilisation et les besoins toujours accrus en effectifs mettent en péril l’efficacité et la crédibilité du processus de sélection. La procédure de l’examen des recrues et les critères de l’aptitude militaire sont considérablement allégés. En conséquence, des recrues qui auraient, en temps de paix, été écartées, sont massivement incorporées . Les 360 000 morts de 1915 et la nécessaire répartition planifiée des travailleurs dans l’économie de guerre conduisent à l’adoption de la conscription en mars 1916 . L’incorporation prend alors un caractère obligatoire pour les hommes valides en âge de combattre. Alors que l’examen médical chargé d’établir l’aptitude des recrues revêt une importance nouvelle, la contestation des méthodes et résultats de la sélection prend une ampleur significative.
Les aberrations d’affectation qui résultent de l’abandon de la rigueur font l’objet d’une âpre dénonciation et rencontrent un écho alarmant dans l’opinion confrontée de manière inédite au caractère obligatoire du recrutement. Le récit de ces injustices sort peu à peu de l’espace de la supplique familiale pour accéder à un plus vaste public. Les années 1916 et 1917 sont rythmées par de nombreux articles de presse évoquant des pères de famille infirmes, forcés de revêtir l’uniforme du fait de l’incompétence ou du cynisme des médecins chargés du recrutement. La Grande Guerre constitue ainsi une mise à l’épreuve sans précédent de la légitimité des experts médicaux dont les décisions et la compétence sont contestées par le corps politique et l’opinion publique avec une grande virulence. Cette crise de la reconnaissance sociale entraîne alors une campagne de réhabilitation réalisée dans une logique de restauration de la légitimité.
Au croisement de l’histoire de l’expertise et de la sociologie des pouvoirs, je m’intéresse ici au devenir et aux stratégies de la profession médicale, aux prises avec les exigences parfois contradictoires de l’armée et de l’opinion publique en temps de guerre. L’analyse s’appuie sur les débats des années 1916 et 1917 à la Chambre des Communes, les documents de la commission d’enquête parlementaire menée par Edward Shortt, la presse médicale et générale ainsi que sur les archives du Ministère du Service national. J’étudie dans un premier temps la crise de la reconnaissance sociale que subissent les médecins du fait de leur mise en cause dans les dysfonctionnements du recrutement (1). J’insiste ensuite sur les modalités de leur mobilisation en analysant les formes et contenus de leur argumentation (2). Je termine enfin en évoquant les dissensions entre médecins civils et médecins militaires qu’a provoqué cette crise (3).

On April 1, 2019, in Bucklew v. Precythe, the United States Supreme Court rejected a  Missouri death-row inmate’s claim that executing him by the State’s lethal injection protocol would violate the Eighth Amendment’s ban on “cruel and unusual punishment” because blood-filled tumors in his head, neck, and throat could rupture, causing him to choke and suffer “‘prolonged’” and “‘excruciating pain.’”  In essence, the Court found that Bucklew failed to establish “a feasible and readily implemented alternative method” of execution because he provided so few facts about nitrogen gas, the alternative that Bucklew suggested.  In its argument against Bucklew’s claim, the State emphasized that experienced medical personnel, including a board-certified anesthesiologist, would be utilized during the execution to ensure  that Bucklew would not face a “substantial risk of severe pain.”  Bucklew’s medical expert countered strongly with compelling evidence that Bucklew would suffer greatly.  Bucklew is far from the first capital case to rely on physicians’ contributions in the execution process.  Indeed, physicians have been participating in executions for more than a century. This participation is particularly troublesome given the wide range of unresolved problems created by lethal injection, including drug shortages and untested drugs and protocols. The American Medical Association and other medical organizations have turned a blind eye to this reality or they have blamed the legal system for the current state of affairs.  This commentary contends that whatever moral or professional credibility the medical community fears it will lose by engaging in a discussion of physician involvement in lethal-injection executions is already imperiled by the increasingly apparent divergence between the community’s words and its actions.
