Objective: To assess knowledge and practices regarding diabetic foot ulcer prevention among diabetic patients attending a tertiary care hospital in northern India, and to identify factors associated with knowledge levels. Methods: A descriptive cross-sectional study was conducted among 209 diabetic patients without current foot ulcers attending the outpatient department at Adesh Medical College and Hospital, Haryana. A systematic self-administered questionnaire measuring knowledge of diabetes, foot ulcer risk factors, and foot care behaviors was used to gather data; knowledge scores were classified as inadequate (less than 50%), average (between 50 and 75%), or adequate (more than 75%). Results: The mean age of participants was 45.6±16.7 years with an distribution amongst male and females. Type 2 diabetes was predominant (83.4%). Despite 75% of participants demonstrating inadequate knowledge about diabetic foot care, approximately 63% practiced reasonable foot care. Most participants correctly knew that feet should be washed daily (79.5%) and with warm water (72.6%), but only 33.2% regularly inspected their feet. Patients receiving information from healthcare professionals demonstrated significantly better knowledge (p<0.01). Educational status, employment, and type of diabetes were significantly associated with knowledge levels. Conclusion: Despite practicing reasonable foot care, diabetic patients demonstrated considerable knowledge deficits regarding foot ulcer prevention. Healthcare provider education plays a crucial role in improving knowledge. Structured educational interventions specifically targeting high-risk populations with low socioeconomic and educational backgrounds are necessary to prevent diabetic foot complications and subsequent amputations.

The study "Analysis of Postoperative Complications in Gastrectomy" evaluates the outcomes and early postoperative complications in gastric cancer surgery. The retrospective study (2015-2021) reviews 253 cases, comparing subtotal vs. total gastrectomy and analyzing factors such as surgical complications, metastases, hospitalization duration, and mortality rates. Findings indicate that subtotal gastrectomy has lower morbidity and mortality, while total gastrectomy is associated with higher complication rates. The study emphasizes the role of multidisciplinary management and early detection of complications to improve prognosis.

Background : The district hospitals in Cross River State of Nigeria serve majority of the population residing in the rural areas and little is known about their capacity to provide anaesthetic and surgical care. The present study is an initial needs assessment of anaesthesia and surgical services in public hospitals in Cross River State as a stepping stone for strengthening anaesthesia and surgical services. Methods : A standardized Lifebox Hospital Initial Needs assessment questionnaire and another structured questionnaire were used to assess the anaesthesia and surgical services in 16 general hospitals (district hospitals). The survey did not involve face-to-face interviews. Results : There was no practicing physician-anaesthetist in the 16 district hospitals in Cross River State. Hibernomas refer to benign tumours of immature adipose tissue known a brown fat. They are most commonly found in areas where brown adipose tissue is still present in adulthood such as the shoulder, back, neck, best, arm and abdomen. This case study describes a 60 year old male who presented with a growing 15 mm lump in the right forearm. On examination, tenderness in the area surrounding the mass was elicited. A magnetic resonance image of the right forearm was performed which reported a large intramuscular lipomatous tumour within the brachioradialis muscle, in close relation to the neuromuscular bundle. Surgical excision was performed successfully. Histology was consistent with a diagnosis of a hibernoma tumour. The patient healed well with no evidence of infection or recurrence on follow up.

Background: - chronic afferent loop syndrome is one of rare complication after Bill Roth II
gastrojejunostomy. Surgical correction is must to avoid complications that come after conservative
management. Most common complications of chronic afferent loop syndrome are sepsis, pancreatitis,
cholangitis and ischemia of the afferent loop.
Case presentation
Here we are presenting 18 years old female patient presented with repeated episodes of bilious vomiting,
nausea, epigastric discomfort and distension after her 90th post-truncal vagotomy and Bill Roth II
gastrojejunostomy for gastric outlet obstruction secondary to peptic ulcer disease stricture. Braun’s
corrective anastomosis done and the patient discharged with improvement. Now the patient was on
follow up with no compliant.
Conclusion
Although rare to happen after Bill Roth II gastrojejunostomy reconstruction, it is better to consider
chronic afferent loop syndrome irrespective of surgery done time for those who have bilious vomiting.
Revision of the bypass surgery is must for chronic afferent loop syndrome to avoid serious
complications.

Background: Cholecysto-colonic fistula (CCF) is the second most common cholecysto-enteric fistula and is often discovered intra-operatively. The current case depicts a rare variety; cholecysto-duodenal (D2)- appendiceal fistula of a 55 years old female patient with a history of
cholecystitis and endometriosis. Cholecysto-colonic fistulae are mostly found in women in their 6th to 7th decade and is rarely diagnosed preoperatively. Yet we report that the current case was evident during CECT abdomen as cholecysto-colonic fistula but intra-operatively precisely found to be dual fistula consisting of 1.) Cholecysto-duodenal (D2) fistula with a stone impacted in between the fistula tract without perforation to peritoneal cavity, & amp; 2.) cholecysto-appendiceal
fistula with appendix fused to gall bladder forming a tract and also adhered from serosal layer to the duodenum. The rare positioning of high located appendix along with recurrent cholecystitis must have may have caused the tip of the appendix to fistulate with the inflamed gallbladder.

Method: Open cholecystectomy, D2 repair and appendicectomy was performed in this patient was unremarkably recovered without subsequent peritonitis or ileus. CCF presents as an acute onset disease entity mostly with biliary ileus; yet this patient presented with typical signs and symptoms of cholecystitis without loose stools or ileus. In 1/4th of such patients with CCF, a second hepatobiliary abnormality is found including gall bladder cancer in 2% cases, but none found with regard to current patient.

Conclusion: CCF presentation may vary from typical cholecystitis presentation to gall stone ileus. Pre operative CECT plays a good role in identifying yet intraoperative findings may be more precise than imaging study conclusions.

Appendicitis has always been considered a clinical diagnosis wherein the surgeon's clinical acumen and laboratory investigations take priority over the radiological imaging. However, an atypical presentation of appendicitis can occur due to the varying location of the appendix which may result in delayed diagnosis and complications. In such cases radiological imaging such as contrast enhanced computerized tomography helps in the diagnosis. We report two cases of subhepatic appendicitis with different clinical findings.

Background and Aim: There is paucity of prospective data on association between cervical esophageal webs and celiac disease. It is not clear whether all patients with cervical esophageal web need screening for celiac disease. Hence, the present study was carried out to determine the association of cervical esophageal web with celiac disease. Methods: This prospective study included consecutive patients with symptomatic cervical esophageal web diagnosed over a period of 4.5 years. Tissue transglutaminase antibody was measured in serum of each patient. Patients with elevated tissue transglutaminase antibody titer were subjected to esophagogastroduodenoscopy and biopsies were obtained from the descending duodenum to look for histological changes of celiac disease. Esophageal web was treated with bougie dilatation. Celiac disease was diagnosed on the basis of elevated tissue transglutaminase antibody and suggestive duodenal histology. Results: Twenty one patients were diagnosed to have cervical esophageal web. Eighteen (85.7%) had evidence of iron deficiency. Five (23.8%) patients with cervical esophageal web fulfilled criteria for diagnosis of celiac disease. All five had evidence of iron deficiency. None of these patients gave a history of chronic diarrhea. All patients were treated with bougie dilatation. Patients with celiac disease were advised of a gluten-free diet. All five celiac disease patients are free of dysphagia without recurrence after a mean follow up of 10 months (range: 3 to 16 months). Conclusions: There is association between cervical esophageal web and celiac disease. All adult patients with cervical esophageal web and iron deficiency need screening for celiac disease even in the absence of chronic diarrhea.

Introduction: Prostate cancer (CaP) is the second most common cancer in men and the fifth most common cancer worldwide. The incidence in Nigeria is rising. Numerous challenges exist that prevent the successful management of these patients in this subregion. Aims and Objectives: This study aimed to report on the modes of presentation and difficulties encountered in managing patients with CaP in our environment with a view to finding solutions to these challenges. Materials and Methods: This was a retrospective study of all CaP patients who were managed in Alex-Ekwueme Federal University Teaching Hospital, Abakaliki, Ebonyi State, Nigeria over a 10-year period from 2012 to 2021. Data were collated using a pro forma. Numerical data were summarized as means, median, and standard deviation, whereas categorical data were summarized as frequencies. Statistical significance was pegged at P < 0.05. Results: Seventy-three patients were analysed. The mean age of the patients was 71.48 ± 8.15 years. The three most common presenting complaints were lower urinary tract symptoms (LUTSs) 23 (31.5%), acute urinary retention 9 (12.3%), and LUTS with low back pain 9 (12.3%). The median duration of symptoms was 6.5 months. No difference was noted among educational level, occupation, and stage of CaP, (P=0.222 and P=0.548), respectively. The median total prostate-specific antigen was 85.0 ng/mL. Sixty-seven patients (91.8%) had an abdominopelvic ultrasound scan. Fifty patients (68.5%) had stage 4 disease. Thirty-eight (52.1%) had financial constraints. Forty-nine (67.1%) patients were lost to follow-up. Bilateral orchidectomy was offered to 28 (38.4%) patients. Conclusion: Financial constraint was a huge barrier in the management of CaP patients in this study. Late presentation was common in this study.

AMAÇ:
Çok yaygın görülen safra taşı hastalığı hakkında ilk tanımlamalar 15. yüzyılda yapılmıştır. Daha önceki dönemlere ait yeterli bilgi bulunmamaktadır. Eski Mezopotamya çivi yazılı metinlerinde safra kesesi hastalığı ve kolelitiazis varlığını araştırmak.
YÖNTEM:
Eski Mezopotamya çivi yazılı belgelere, 2005 yılında Scurlock tarafından yapılan modern hastalıklara göre metinlerin sınıflandırıldığı çalışmadan ulaşıldı. Safra kesesi hastalığı ile ilgili olduğu düşünülen altı metin seçildi. Metinler ZÉ.GİG, ZÉ (safra kesesi hastalığı) ve Pašittu (kolelitiazis) kelimelerinin varlığı ve safra kesesi hastalığında görülen beş ana semptomlarını içermelerine göre seçildi. Bu metinler güncel tıp literatürleriyle yorumlandı.
BULGULAR:
Çalışmada ZÉ.GİG, ZÉ ve pašittu kelimeleri beş metinde geçmekteydi. Bu metinlerin üç tanesi 2/5 (0.4), iki tanesi 4/5 (0.8) semptom içermekteydi. ZÉ.GİG, ZÉ ve pašittu kelimelerinin geçtiği bu beş metin arasında bir fark olmadığı (p = 0,519 ) ve güncel safra kesesi hastalığı kliniğini betimledikleri sonucuna ulaşıldı (sırasıyla p = 0.167, p = 1.000) ). Hastalık ismi içermeyen 6.107 nolu metin ise 4/5 (0.8) semptom içermekteydi. İstatiksel olarak p = 1.000 değer ile safra kesesi hastalığı kliniğinden farklılığının olmadığı tespit edildi. Ayrıca 6.99 nolu metinde ayırıcı tanıda peptik ülserin (tugānu) göz önünde bulundurulduğu tespit edilmiştir.
SONUÇLAR:
ZÉ.GİG, ZÉ ve pašittu kelimelerinin safra kesesi hastalığı kliniğiyle ilişkili olduğu bulundu. Çalışmaya alınan Eski Mezopotamya çivi yazılı altı metnin safra kesesi hastalığı ve kolelitiazisle ilişkili olduğu tespit edilmiştir. Bu çalışmayla safra kesesi hastalığı ve kolelitiazis tarihçesi, MÖ 1. binyıl metinlerine, hatta Sumer dönemine kadar uzanacaktır.

Epidemiological surveillance is an essential component of public health systems worldwide, serving as a cornerstone for effectively managing health issues by monitoring disease distribution and detecting potential epidemics at their earliest stages. In Djibouti, a country situated in the Horn of Africa, the National Institute of Public Health plays a crucial role in overseeing the surveillance of 35 notifiable diseases, meticulously analyzing trends, and confronting the challenges posed by emerging health threats. This literature review delves into the comprehensive analysis of epidemiological data from the year 2023, with a primary focus on identifying prevalent trends, assessing geographical distributions, and elucidating demographic patterns. Among the diseases frequently reported during this period were malaria, influenza-like illnesses, and acute diarrheal diseases, indicating their significance in the local health landscape. Data collection was conducted every week, employing a combination of active and passive surveillance methods across a network of healthcare facilities throughout Djibouti. Active surveillance involves proactive efforts to seek out cases through systematic monitoring, while passive surveillance relies on the voluntary reporting of cases by healthcare providers. The utilization of both methods ensures a comprehensive and nuanced understanding of disease dynamics within the population. The findings of this review underscore the importance of ongoing surveillance efforts in Djibouti and offer valuable insights into potential areas for improvement. Recommendations aimed at enhancing surveillance effectiveness include strengthening data collection mechanisms, improving reporting systems, and enhancing collaboration between healthcare facilities and public health authorities. By implementing these recommendations, Djibouti can further fortify its capacity for disease surveillance and response, ultimately leading to improved public health outcomes and the mitigation of health threats for its population. This study contributes to the broader discourse on epidemiological surveillance in resource-limited settings and underscores its critical role in safeguarding public health.
