Thanks for the code, but sorry it didn’t work. I added the code snippet and tried to delete the widget and re-added it in a sample form and still it doesn’t appear either in preview mode or in the page.
Tried with a new API key and still the same.

The widget doesn’t appear even in the preview form or on the site. And gets this error message when trying to send – https://prnt.sc/rzHNA038hLEu
I tried to create an entirely new form and added the widget, and no success.

Is there anyway to find out any information at all from logs or anywhere so that I could ask the particular customer to resubmit?

Since someone had a successful hit to my wp-admin/xxx does that mean they had gained access to the site ?
(I tried to recreate the issue by hitting the same url, and is only seeing blank page.)
When in Protection mode, Wordfence blocked this request, which is great.
Those successful hit to /wp-admin/xxx was during learning mode.Thanks!
