Facing the same issue. Was working fine previously.

On checking with the host, was advised to provide direct PHP file access. This is a less secure option, but the PDF is displayed when access is provided.

My mistake ‘Unspecified’ refers to the shipping class label.

Few suggestions:
- At present the shipping rate column is labelled as ‘Unspecified’ – This can be updated to – Enter rate per Kilometers or miles (similar to Charge per Kilometers checkbox)
- Fill with 0 (zero) can be updated to ‘Enter 0 for zero charge per Kilometer/ miles‘
- Leave blank to disable shipping rate calculation

Misinterpreted the instruction on the page: ‘Fill with 0 (zero) to set as free shipping. Leave blank to disable.’
