Can you try disabling, or if possible reset the plugin as it was out of the box, Really Simple Security Pro and see if this fixes your problem.
Hi Takayuki, I found the issue. I also have another plugin that I completely forgot about. The problem was because of the plugin WP Hide somehow was interfering with the form. After resetting the plugin, the forms started working again.
