Line: ticket_creation_mode varchar(50) NULLIt needs a , (comma) at the end or it fails the SQL and the tickets table is not created. add that , before you activate the plugin and it will create all the tables and run fine for you.

If you update to latest version 1.1.5 (just updated)

Where are you getting that error? I just Installed the plugin on a fresh wordpress install and could not replicate that error.
