Did you re-activate entrywizard? Were there any error messages when you did?
I’ve just tried completely removing then re-installing EntryWizard with no error messages. The back-end control panel looks fine, but when I try to display just the example webform, I still see the short-code, which is not being translated into the form. Before the update to 1.3.21 (and PHP7.4) everything was working OK.
I had a few issues with getting the upgrade completed successfully, then when I did lost all my historic rating forms unfortunately. Haven’t figured out how to get them back. As everything is downloaded, have called it a compulsory cleanout which was probably overdue!
If you did not make any changes in entrywizard since it last worked,
it could be your theme — try a different one (one that supports PHP 7.4) and see if that makes any difference.
I’ve tried installing on a different website with a different theme – same problem. I then created a completely new WordPress site – no theme or any other plug-ins, just installed WP (v5.8.2), loaded the EntryWizard plug-in and created a page to display the example webforms, and again it displays the short code rather than the form. So no conflicts with other plug-ins or themes.

OK, now sorted – I knew it would be something stupid. When there was the PHP problem with 1.3.21, I temporarily removed the webform shortcode from my website page. I then re-instated the short code by cut/paste from the webform management area. However this short code has two spaces between show_webform and identifier, so doesn’t get translated. At the next update, please check the example short code on the webform management page to ensure it can be directly cut/pasted.

Shortcode format rules are very strict, which is why I added the drop-down in the classic editor, but I never noticed that extra space in the text at the top of the webforms page.
