Username (unknown)

Looking at the list of successful logins on one of my sites I see a few lines where the username is “(unknown)” — in parenthesis. Judging by the IP numbers it could be me but I can’t say for sure (don’t use a static IP).
Should I be concerned, very concerned, or is there a logical explanation?
