Thank you for bringing that to my attention. I was able to replicate that problem and I will work on a fix for this.
I was just testing and I saw that the format is shown correctly with the datepicker date type! So while I am working on the fix, you could change the field to a datepicker type datefield to temporarily fix this in the form for your users.
The fix will be released as a new update in a few days. I will notify you here when that happens.

This problem has been fixed since version 1.1.2 which was released a few minutes ago.
Please update the plugin and remove the cache from the site and browser so that you get the newest javascript file.
After that everything should work as intended! Do let me know if that is not the case.
