Unable to create application | WordPress.org

I have just loaded the DB Toolkit and when I try to create a new app the entire screen is gray I am able to enter an application name but not a description. When I click the create application button nothing happens. When I click on the cancel button nothing happens. I have to use the browsers back button to get back to the WordPress dashboard. I have tried using IE, FireFox, and Chrome. Is this a compatibility issue with 3.6.1? I would really like to see if your plugin will solve my database needs. Thank you

I noticed that too, but was able to get around it by tabbing (keyboard tab key) over to the Create Button and then click to create.

But, I am stuck on the create interface – turning on the field visibility icon. There am getting hit with

ReferenceError: df_setToggle is not defined

It is the same error for all the field property icons.

I am playing around on an hosted server and is not local.
