Ultimate Member Account form not working

Hi, I’ve been struggling over this for some time now. I am using ultimate member (free version) and 2017 theme.
Everything is working fine and I can see and use all forms, except the account page. If I post the account form code [ultimatemember_account] on any page it causes that page to not display correctly and even somehow makes the wordpress admin bar disappear from the top of the page. This is the one form that is not easily editable so I don’t know where to start. I have tried uninstalling ultimate member and reinstalling, still not working. Before I try changing theme I wondered if anyone ha any suggestions?

Resolved.
If anyone else has this issue check your um settings > account theres an on/off switch to add a first name and last name, mine was off. When i turned it back on i found 2 more options showed up and one being: disable first and last names, this was switched on, so even though I could’t see this option, they were causing a conflict. I hope i can save someone else a headache by sharing!

Tried theme/plugin conflict test – no dice.
I read in a previous form it could be: regarding a php extension or require a php update? I installed the Query Monitor and have no errors.Profile page works great, when I hit the cog icon or the add to your profile link the content disappears. I’ve checked the console and the html is not there for the form.
