It seems that the problem is solved, by going to the plugin settings > advanced > heading levels, selecting the headings you want to display (H1,H2,H3,…) and save the settings.
I had the same issue. The plugin stopped working after an update. But it looks like the reason is that the settings got wiped. I went to Settings > Advanced > reselected the “Heading levels” and clicked “Update Options. This fixed it.

I followed your steps but it still doesn’t fix the problem. On my blog it only shows a blank rectangular box with no TOC. Any ideas?
Double check, if Settings → Advanced → Heading levels matches your actual heading set in your blog.
I had an issue where I had checked that ToC must be build based on H1 and H2 only while that particular blog was using H3 and H4 only.
