I am using it with the latest WP version.

The plugin must get the data from the database to determine the post where it should go, so there will be some server load. But that’s normal for plugins. The Plugin will only work, if someone accesses the URL with the parameter redirect_to. Otherwise it is doesn’t do anything.

?redirect_to=… redirects with status code 302. Search engines can therefore recognize that this is not just another URL with duplicate content.
