Could you please share the form export so that we could give a closer look?

If you are concerned about any sensitive information in the form, then you can duplicate your form, remove any sensitive information, and then export it.

You can share the export file via Google Drive, Pastebin, Dropbox, or any cloud service in the next reply.

I ran some tests in my lab site using the form export you shared and the latest version of WordPress and Forminator but couldn’t replicate this issue.

It seems something on your site is not compatible with the new WordPress version and is causing issues with Frominator styles.

I would suggest making sure all of your other plugins and theme is updated.
