Quiz Issue

Not able to see the Quiz on the front end of the site. I can see it when editing.

I tried the following guide from GitHub, but it didn’t work.
— In quiz settings, enable ‘Pass required’.
— Enable ‘Random question order’ and set a number to ‘Number of question’ that appears.
— Click update.
— Possibly revert to the original settings.

Getting the following error on the website …
Warning: Cannot modify header information - headers already sent by (output started at /home/customer/www/MY WEBSITE/public_html/WEB FOLDER/wp-config.php:1) in /home/customer/www/MY WEBSITE/public_html/WEB FOLDER/wp-admin/includes/misc.php on line 1381

In Chrome’s Developer Console, I see the following error …

JQMIGRATE: Migrate is installed, version 3.3.2
DevTools failed to load source map: Could not load content for MY WEBSITE/sm/fb3ed351cd5c0f1f30f88778ee1f9b056598e6d25ac4fdcab1eebcd8be521cd9.map: HTTP error: status code 404, net::ERR_HTTP_RESPONSE_CODE_FAILURE

Note: I am reusing the same questions for a PreTest as I am using for the actual graded Test. Not sure if this could be causing a conflict?

I’m using Sensei LMS Version 4.6.3
Sensei Pro (WC Paid Courses) Version: 1.6.1
PHP & WP is current
