Are the leads showing up in your CRM, or is nothing being recorded at all?
Where are you seeing the “sample notification”? In your email, or somewhere in the dashboard?
What type of notification are you expecting (email, in-app, etc.), and which email address should be receiving it?
If possible, could you submit a test entry now and let me know the exact time? That way I can trace what’s happening on our end. You can share screenshots if that helps. You can create screenshots using something like Snipboard or Imgur and post the link here.
In the meantime, it’s worth checking your spam/junk folder just in case the notifications are landing there.
