I am not sure you have checked these options in the plugin settings page (see screenshot). The settings are “Enable category filters” and, if enable, you can choose between showing the categories as a series of checkboxes or as a dropdown.
If you have correctly configured these settings and yet you don’t see the categories, you can also try to delete the plugin’s transient cache, it is a button near the end of the settings page labeled “Clear store locator transient cache”.
And last but not least, double check if you are using a custom template for the store locator itself, you may have omitted the html markup where the categories are supposed to be displayed, although on a first look it didn’t appear as if you are using a custom template, but just in case.
Please try some of the above and get back if you are still facing this issue.

I looked at your demo page, but it didn’t work on the Japanese map. It may be different from what I want to do. I just delete my page.
Thanks for your help. Now I could understand the functionality of this plugin. I was wondering if Google would automatically pick up some stores when I set keywords in the plugin. I found that with this plugin, I had to manually register each store one by one. I would like to manually register each store one by one and take the time to create content.
