nav-menu.php hacked

I have weekly the includes/nav-menu.php hacked with injection code and it changes to 444 the permissions of htaccess.
This is the same problem that was treated in this topic:

I would like to find the root solution. The server? Anybody with the same problem or some clue?
I have everything updated. I dont use the Revslider or W3 Total Cache. I use a theme called Bloom from FameThemes.
