    Thanks for reaching out and for the screenshots!

    The formatting issue is caused by the label on the Name field. To fix it, select the Name field in the form builder, open its Advanced options, set Hide label, set the size to Large, and use CSS Classes to place it in a two-column layout.

    For reference, I recommend checking our guide on multi-column layouts with CSS classes.

    A couple of additional notes based on your screenshots:

    • I can see Cloudflare Turnstile is enabled on the form. If you’d like to display the form in a single line, you’d need to set Turnstile to Invisible instead of Managed in your Cloudflare settings, as the Managed widget can break the layout depending on page width.
    • Make sure the Name field is set to Simple format rather than First Last, as the two-part format can also cause visual issues in this layout.
