Same has happened to my site. The problem looks to be centred on conditional fields and whether they contain a value or not. I have a subscription form that has worked for several years until the latest Forminator release on 10th July.

Unfortunately, there’s unexpected bug in new release that causes it. It is, indeed, related to conditional fields (specifically – conditionally visible fields that are set to be required) on pages forms.
Our developers are already aware of that and are working on a fix. I don’t have ETA yet but it will be released soon as it’s quite a critical issue.
I apologize for the problem it causes. For now, the workaround is – as you already did – downgrade to previous release.

Thank you for your response. I was able to introduce a workaround to the issue for my subscription form. Our member renewal period started on the 1st July, so this was important!
However, an occasional issue is to be expected from all software developers. The free version of Forminator is incredible value for money!
