@madtownlems do you use some role manager plugin in your WordPress site?
Because I use User Role Editor Pro and I suspect it to be the cause of this conflict with WP Media Library.

Nothing off-the-shelf, but we do have custom permission-filtering code.
I planned to spend some more time this morning trying to isolate what other plugin(s) might be related to the behavior, but this time I couldn’t reproduce the issue again.
I totally understand why it would be difficult for the Plugin Developer to track down the issue; it has appeared and disappeared incredibly sporadically. If/when it returns for me again, I’ll spend more time trying to make it more consistently reproducable.
