Issue with WCPA Form Not Displaying

    I’m facing an issue with the WooCommerce Custom Product Addons plugin. I’ve set up multiple WCPA forms, and they’re all working perfectly — except for one specific form with the ID 2688.

    Here’s a summary of what I’ve tried so far:

    I uploaded the form using a CSV file with values structured identically to other working forms.

    I confirmed that all settings and positions are correct.

    I even duplicated a working form, updated the fields, and saved it under a new name, but the duplicated version still does not display on the product page.

    I also tried adding the form manually without the CSV, but nothing shows up.

    Despite all these steps, the form with ID 2688 refuses to load or appear. Could you please help me troubleshoot this issue?

    Upon checking the product page, we can confirm that some fields are indeed being loaded from the form with ID 2688. If you are experiencing missing fields or parts of the form not displaying as expected, kindly specify which fields are not appearing.

    Additionally, please share the exported form file so we can review the configuration and identify any potential issues more effectively.
