Here’s one possible way to do this with Forminator:
Create a unique URL with a query parameter for each of your representatives to use: ex http://localhost:10008/sample-page/?ref_code=888

Then create a Hidden Field in your form with these setting – https://i.imgur.com/0lO6UfK.png

This field will capture the “ref_code” from the URL and submit it along with the data your user filled in the form.
Example Submission: https://i.imgur.com/nW9FRN3.png
