In the console, I can see an error related to the LiteSpeed caching plugin, so we assume it’s causing the issue. Please bypass the form page from caching or deactivate the plugin temporarily.

.If the issue persists even after deactivating the caching plugin, please check this documentation guide to identify any plugin conflicts, as there is no such issue in our environment. Conflicts are common on WordPress platforms.
