Getting Debug Notice and Dropdown doesn't format

I installed WooCommerce Admin Bar Addition on my multi site and I got the following notice while I had debug turned on.

Notice: Undefined variable: menu_items in /mysite/public_html/wp-content/plugins/woocommerce-admin-bar-addition/woocommerce-admin-bar-addition.php on line 592

Also the drop down doesn’t actually format properly after it was installed. Screenshot
