Sure. I might be going mad, but how do I do that in this forum? If I try and paste an image it doesn’t work and I can’t see an attachment option – am I doing something wrong?

You could host the image yourself by uploading it to your WordPress site (media) and pasting the link here. You can also use a 3rd party service like http://www.imgur.com .
Now that’s odd. I would say it is a CSS issue, but…
the ‘Ed’ part of ‘Edit’ is outside it’s table column. I don’t think that’s a css issue but rather a broken DOM. Unfortunately I’m not a css / design expert. Can you use the element inspector of your browser to see if the DOM is ok? I.e. the contents of every cell should be within a <td>
This is probably caused by a plugin (or theme) that messes up the columns of your orders page. FireFox has a style editor which allows you to disable certain css on-the-fly maybe that can help you finding the issue.
It’s also possibly a minification issue. Do you have minification plugins enabled?
