It sounds like the URL to your “custom withdrawal form” is incorrect. You could either correct the URL to your own form, or use the default Complianz withdrawal form instead:
Complianz -> Terms & Conditions -> 2. Questions -> Do you want to use a custom withdrawal form -> “No.”

I also have this issue. I don’t use a custom form and ‘No’ is ticked. This is the default link – https://www.wellbeingbristol.co.uk/wp-content/uploads/complianz/withdrawal-forms/withdrawal-form-en.pdf
The php file exists – /wp-content/plugins/complianz-terms-conditions/templates/withdrawal-form.php/ and I have not edited it.

It sounds like the file withdrawal-form-en.pdf
is not present on your server, so you receive a 404 page instead.
I would try the following, navigate to: Terms & Conditions -> 2. Questions -> Communication -> “Do you want to limit the interpretation of this document to your current language?”, and switch it to the option opposite of your current selection.
Save settings after changing the option, and after doing so, change it back to the option you originally had it set to and Save settings again.
That should result in the withdrawal-form-en.pdf
file being re-generated in the /wp-content/uploads/complianz/withdrawal-forms/
folder.
