We are currently investigating a case where something similar is happening. What we found out for now is that not only the header is missing but also the products that should be in the first batch.
In this particular case it seems to have happened when the plugin ran into a server configuration limitation. It got stuck on processing the first batch and as such is also missing the header. We could solve it by lowering the batch size from the default 750 products per batch to a much lower batch size of 100. Could you try if this would also solve the issue at hand for you?
