Choosing the right question type

I would like to create a quiz with the following specifications :

– Several pages (10)

– On each page, first a title and then, just under it, a description of some mathematical context (e.g. : let f be the function defined by … blah blah )

– Under that description there are several affirmations (let’s say 3 affirmations). Each affirmation can be true or false and the user has to check the right button (true / false) or optionally to leave the buttons unchecked if he/she doesn’t know.

– checking the right button gives 1 point, checking the bad one gives -1 point and if no button is checked, then the user gets 0 point.
