Checkout Form menu missing from WooCommerce Dashboard menu

    We are currently running this plugin in a multisite installation. From the admin dashboard (and for most of the sites), we can see the ‘Checkout Form’ menu option under the WooCommerce menu but there are a couple of sites where the menu is omitted. I’ve tried to go through the WooCommerce-Settings to see if we have inadvertently disabled something but I’ve been unable to find anything that would explain why this menu item is missing.

    I have check and confirmed that the latest version of the plugin is installed and it is running as ‘Network Active’.

    What would cause this menu item from being omitted?
